This hourly contract position is perfect for a practicing LPN looking for additional flexible hours. The schedule is flexible, and hours will vary weekly. and can mostly be scheduled at the LPN's convenience.

Under the supervision of the RN, the LPN will impact lives daily by conducting client assessments and working with caregiving staff to ensure proper delivery of care.

Responsibilities of the LPN:

  • Travel to clients' homes for assessments/visits as needed, at the direction and in consultation with the RN.
  • LPN will be located in Lafayette, LA client homes in Region 4
  • Assist RN with training Direct Support Workers (DSW) on medication and non-complex treatment administration within the guidelines of LDH, LSBN, Medicaid and New Day. Track ongoing staff training to ensure that DSWs meet licensing requirements.
  • Assist RN with providing safe and proper medication administration and treatments.
  • Assist RN with oversight of in-home medication and non-complex treatment administration by DSWs.
  • Track ongoing staff training and competencies to ensure that DSWs have met all requirements to perform non-complex tasks and administer medications safely within LDH, Medicaid, LSBN, and New Day Personal Care Services guidelines.
  • Schedule and track clients’ doctor appointments and communicate scheduling information to other departments. Arrange for transportation as needed.
  • Log received medications, check for accuracy, reconcile medications and coordinate home delivery on time.
  • Review/Create Medication Administration Records (MARs) and medical delegations (MedDels)other required nursing documentation for completion, accuracy, and signatures. Prepare and maintain client medical binders and records.
  • Ensure that clients' health and well-being are maintained. Report health status changes to RN and other members of client care team immediately upon discovery.
  • Communicate frequently with clients, families, and other affiliates to answer questions, resolve issues, and ensure satisfaction with services.
  • Properly document all client communications, care plan or medical changes, visit notes, or incidents.
  • Comply with all program guidelines and meet requirements established by LDH, Medicaid, LSBN, and New Day Personal Care Services.
  • Promote New Day PCS in the local community and build relationships with external stakeholders
  • Model the New Day PCS values of Compassion, Trust, Integrity, and Accountability

Requirements:

  • Must have a valid Louisiana LPN License.
  • 2 years experience preferred (patient care/medication management/case management)
  • Must possess strong Administrative and Clinical Skills
  • Effective Communication using various forms of technology, including Zoom, Skype, email, and phone.or
  • Basic Proficient computer skills with experience using Microsoft Office programs, Word, Excel and Outlook.
  • Satisfactorily pass all required and applicable State & Federal eligibility verifications, including background, driving, and other checks.
  • Valid driver's license, auto insurance, and reliable transportation

 

Job Types: Part-time, Contract, PRN, Travel nursing

Pay: From $28-$34 per hour
