This hourly position is perfect for a practicing LPN looking for additional flexible hours. The schedule is flexible, hours will vary weekly, and visits can mostly be arranged at the LPN’s convenience. Under the supervision of the RN, the LPN will make a daily impact by conducting client assessments, managing medication needs, and supporting caregiving staff in the delivery of quality care.
Responsibilities of the LPN:
Travel to client homes in Region 4 for assessments and visits as directed by the RN.
Assist RN with training Direct Support Workers (DSWs) on safe medication administration and non-complex treatments within LDH, LSBN, Medicaid, and New Day guidelines.
Provide oversight of in-home medication administration and treatments.
Track staff training and competencies to ensure compliance with licensing requirements.
Schedule and track client doctor appointments; arrange transportation as needed.
Log received medications, check for accuracy, reconcile records, and coordinate timely home delivery.
Review and maintain Medication Administration Records (MARs), medical delegations, and other nursing documentation.
Ensure client health and well-being; report any status changes to the RN and care team immediately.
Communicate frequently with clients, families, and affiliates to answer questions and resolve concerns.
Appropriately document client communications, care plan changes, visit notes, and incidents.
Comply with all program, state, and federal requirements.
Represent and promote New Day PCS in the community while modeling our values of Compassion, Trust, Integrity, and Accountability.
Qualifications:
Valid Louisiana LPN License.
Minimum 2 years of nursing experience preferred (patient care, medication management, or case management).
Strong administrative and clinical skills.
Proficiency in EMAR and Microsoft Office (Word, Excel, Outlook).
Effective communication skills, including use of phone, email, Zoom, or Skype.
Valid driver’s license, auto insurance, and reliable transportation.
Must pass state and federal background, driving, and eligibility checks.
